The Lisfranc ligament is one of several ligaments connecting the medial spiky to the second metatarsal. This relationship maintains proper alignment between the metatarsal bone and the tarsal bone. In 20% of people there are two bands of this ligament (back and plantar). Occasionally, the latif ligament refers specifically to the ligament that connects the superior, medial lateral surface of the medial cuneiform to the superior, medial surface of the second metatarsal base. It was injured or disturbed in a Lisfranc fracture. Trauma to the midfoot is caused by direct and indirect impact forces. Direct force involves objects that land on the surface of the foot while indirect forces involve twisting the legs, usually impacting to the heel while the foot is pointed downward to the ground. The mild form of this injury results in widening of the gap between the first and second metatarsals. The extreme form of the Lisfranc fracture causes a complete dislocation of the metatarsal from the tarsal bone. When the midfoot alignment is affected by joint cartilage, it breaks quickly.
