Plot

In 2002, former Pittsburgh Steelers center Mike Webster was found dead in his pickup truck, after years of mutilation and homelessness. Before his death, a football player, Justin Strzelczyk, came to him, and confessed that he was beginning to lose his memory, that he was saying strange things to his children, and almost threw his wife to the wall. Omalu scrutinized Webster's brain microscope slides and found that he had severe brain damage. He eventually determined that Webster died as a result of the long-term effects of repeated blows to the head, a disorder which he later called chronic traumatic encephalopathy (CTE). With the help of former team doctor Steelers Julian Bailes, fellow neurologist Steven T. DeKosky and regional coroner, Cyril Wecht, Omalu published a paper on his findings, which was initially rejected by the NFL.

Over the next few years, Omalu discovered that three other former NFL players, Strzelczyk (died 2004), Terry Long (died 2005), and Andre Waters (died 2006), had symptoms very similar to Webster. He eventually persuaded newly appointed NFL commissioner Roger Goodell to allow him to present his findings before the player's security committee. However, the NFL did not take it seriously, and he was banned from committee meetings, forcing Bailes, a former NFL employee, to give presentations in his place. However, the meeting was set up, in which the NFL claimed that the head trauma of the player was due to a past injury, had nothing to do with football. As he leaves the meeting, the former NFL Player Association executive, Dave Duerson, angrily faces Omalu and tells him to "return to Africa".

Omalu is faced with considerable pressure to withdraw from his efforts, as football is a well-loved sport in Pittsburgh, after giving jobs and allowing men to go to college. Wecht is subject to politically motivated prosecutions for alleged corruption, and Omalu is forced to leave Pittsburgh soon afterwards, lest he be deported, or sent to jail on minor charges as punishment for tarnishing the NFL. Before leaving, he urged the NFL to tell the truth. Omalu's wife, Prema, suffered a miscarriage after being followed. Omalus was forced to leave their dream home outside Pittsburgh. They moved to Lodi, California, where Omalu worked in the San Joaquin County coronary office.

Three years later, Omalu was proved when Duerson committed suicide because of growing cognitive problems. In a suicide note, Duerson admits that Omalu is right. Omalu is allowed to handle NFLPA conferences in concussion and CTE. She told them that she had hoped she never knew Mike Webster, but by knowing her, she had a responsibility to inform NFL players about the true risks they were taking by playing. He says that he has no grudge for the NFL and tells them to forgive themselves and make peace. Amid growing scrutiny from Congress, the NFL was forced to take the issue of a concussion more seriously, and in 2011, NFL players demanded the league for not informing them of the risks of CTE. Omalu was offered the job as head of the medical examiner for the District of Columbia, but as described in the closing title card, rejected him, remained with Prema and his two sons in California, where he became a US citizen in February 2015.
