The Revised Trauma Score ( RTS ) is a physiological assessment system, designed to be used on the patient's initial vital signs. A lower score indicates a higher level of injury severity.

Use in triage
The revised Trauma score consists of three categories: Glasgow Coma Scale, systolic blood pressure, and respiratory rate. The score range is 0-12. In the START triage, patients with a RTS score of 12 are labeled pending, 11 are urgent, and 3-10 are immediate. Those with a RTS under 3 are declared dead and may not receive certain treatments because they are highly unlikely to survive without a significant amount of resources.

These three scores (Glasgow Coma Scale, Systolic Blood Pressure, Respiratory Rate) are then used to take the weighted sum by RTS = 0.9368 GCS 0.7326 SBP 0.2908 RR. The value for RTS is in the range 0 to 7.8408. RTS is highly weighted toward the Glasgow Coma Scale to compensate for major head injuries without multisystem injuries or major physiological changes. RTS threshold & lt; 4 has been proposed to identify patients who should be admitted to a trauma center, although this value may be somewhat low.
