TPLO , or tibial-leveling-leveling osteotomy , is an operation performed on dogs to stabilize rigid joints after the rupture of the skull cranial ligament (analogous to the anterior anterior ligament [ACL] humans, and sometimes colloquially called alike).

In most dogs, cranium ligaments (CrCL) rupture as a result of long-term degeneration, in which the fibers within the ligaments weaken over time. The exact cause of this is unknown, but genetic factors may be the most important, with certain descendants tending to (including Labradors, Rottweilers, Boxers, West Highland White Terriers and Newfoundlands). Supporting evidence for genetic causes is primarily obtained by the assessment of the family line, coupled with the knowledge that many animals will rupture CrCL in both knees, often relatively early in life. Other factors such as obesity, individual conformation, hormonal imbalances and certain inflammatory conditions of the joint can also play a role.

The skull cruciate ligament runs from the center of the skull from the tibial interondylar eminence to the lateral condyle of the femur. Normally, CrCL prevents covert movement (retreat) from Femur relative to Tibia. Due to selective breeding, the tibial plateau slopes have become too tilted backwards so there is constant pressure on the Cruci ligament of the skull. Over time this leads to degenerative rupture. When ruptured, the joint becomes unstable which causes pain and can lead to chronic progressive arthritis if left untreated.

In the TPLO procedure, the tibia plateau, the tibia part adjacent to the stiff, is cut and rotated so that the slope changes to about 5 degrees from the horizontal plane. This prevents the femur from sliding down the slopes of the tibia plateau when the dog puts a load on his knee. Thus surgery generally results in a faster recovery time compared with other procedures to stabilize the knee. Most dogs (more than 90%) are expected to regain a very active and athletic lifestyle with no post-operative complications and without the need for long-term painkillers.
